After giving the background the once over, it looks to me like the volcano is intentional, but the rest is not. Top Left: The volcano appears behind Mario, but I think it's supposed to be scenery. Top Right: You can see some more lava here Bottom Left: See? It just looks like clouds. Bottom Right: Here, Mario is looking right between the "eyes" in the clouds.
