Yes both parties have to add each other. Also you can hold up to 32 friends in your roster per character.

A quick and easy way to add people you don't know, is if you are in one of your friends towns and their friends (who you haven't added) are visiting, you can go to the screen that shows all the players in town (heart icon) and add them by sending a friend request by clicking a button.
