It looks like an unused form of Jr. Troopa. I don't recall this form showing up. For reference, here's all the forms you fight:

1. Normal (Prolouge)
2. Defense (Post-Chapter 1)
3. Flying (Post-Chapter 3)
4. Spiked Flying (Post-Chapter 5)
5. Magic (Chapter 7)
6. Defense, then Spiked Flying, then Magic (Chapter 8)

At no point do you fight him with just the spike on his head.
